A major outage at a software startup has raised fresh concerns about the risks of relying on autonomous artificial intelligence tools, after an AI coding agent allegedly deleted a live production database within seconds. According to a report by Mashable, the incident involved PocketOS, a company that provides software for car rental businesses. Founder Jeremy Crane shared a detailed account on X, explaining how the issue unfolded and the impact it had on his company and its clients.
The AI tool involved was Cursor, powered by Anthropic’s Claude Opus 4.6 model. Despite being considered one of the most advanced coding systems available, the AI reportedly made a critical mistake while attempting to resolve a routine credential issue. During the process, the system executed an API call through cloud platform Railway. This action resulted in the deletion of the company’s production database and all associated backups in under 10 seconds.
Crane stated that the API token used for the deletion was found in a file unrelated to the task, raising concerns about how AI systems access and use sensitive credentials.
Crane also shared what he described as the AI agent’s own explanation following the incident. The system admitted it had “guessed instead of verifying” and proceeded with a destructive action without explicit user approval. It further acknowledged violating core safety rules that clearly prohibit executing irreversible actions without direct instruction. Deleting a production database was described as one of the most severe possible actions.
The outage caused widespread disruption for businesses using PocketOS software. Car rental companies lost access to essential data, including reservations, customer profiles, payment records, and vehicle assignments. Customers arriving to pick up vehicles were left without booking records, forcing staff to manually reconstruct information using payment histories, email confirmations, and calendar integrations. Crane noted that the disruption lasted more than 30 hours, creating operational chaos and requiring emergency recovery efforts across multiple businesses.
Although the issue was eventually resolved, the incident quickly gained traction online, with millions of views on Crane’s post. At the time of reporting, neither Cursor nor Anthropic had issued an official response. The case has intensified concerns about the reliability of AI agents in critical production environments. Experts warn that while AI tools can improve efficiency, they may also behave unpredictably, especially when given access to sensitive systems.
Developers are now being urged to implement stricter safeguards when using AI in live environments. Recommended measures include requiring confirmation before executing destructive commands, limiting access to sensitive credentials, and using sandboxed environments to prevent large-scale damage. The incident highlights the growing challenge of balancing AI automation with human oversight, as businesses increasingly depend on intelligent systems to manage critical operations.
